- How often it happens: [daily / weekly / monthly / ad hoc]

- Tools or systems used: [software, forms, equipment]
- Constraints: [compliance requirements, safety rules, or anything critical to include]
Instructions:
- Use plain language — assume the reader is new to the role
- Be specific enough that someone could follow the steps without asking questions
- If any field above is missing, make a reasonable assumption and proceed
Output format:

2. Scope — who this applies to
3. Step-by-step instructions (numbered, clear action verbs)
4. Roles and responsibilities (simple table: Role | Responsibility)
5. What to do when something goes wrong
6. Document owner and suggested review date
